MassDEP announces revised Title 5 Inspection Form

MassDEP will be posting a revised Title 5 Inspection form and a revised section to the Title 5 Inspection Guidance. This revised form and the revised guidance language will be posted on MassDEPs website by July 31st. These documents are attached to the email for your reference.

During the Title 5/Groundwater Stakeholder Group discussions DEP was asked to add water treatment units to the system inspection form and a place for the inspector to note where the unit discharged to. These revisions have been made to the form in Part D. System Information for both residential and commercial (D.1 and D.2).

No other changes were made, except to clean up the formatting of the form.

For the guidance, revisions were made to the section on Further Evaluation by the Board of Health. These revisions clarify when an inspector can take action and what action can be taken; and, when the inspector cannot take action and has to wait for further evaluation from the Board of Health.

DEP wanted to make you aware of these changes prior to posting the documents to our website. Please feel free to share this information with your respective groups.

Please contact me if you have any questions on the revised form and guidance.

Sincerely,

Marybeth Chubb
